2027 Event SiteThe next phase of obesity treatment may be less about maximizing the number on the scale and more about improving the quality of weight loss. Moderated by Jason Gerberry and Alec Stranahan, the discussion with Caroline Apovian MD and JoAnn Manson MD, DrPH focused on what patients are losing along with fat as GLP-1-based therapies produce increasingly substantial reductions in body weight.
Muscle mass, bone health, strength, physical function and quality of life were recurring concerns. The panel emphasized that changes in lean mass should not be interpreted in isolation. Researchers still need to determine when muscle loss translates into meaningful functional decline, which patients are most vulnerable and whether therapies designed to preserve muscle actually improve strength, mobility and daily life.
The conversation also kept lifestyle in the picture. Resistance training, adequate protein intake and other behavioral strategies may remain important even as pharmacologic treatment becomes more effective. Older adults, frail patients and other high-risk groups will need particular attention as the evidence base expands.
New combinations and mechanisms may eventually help preserve lean tissue while maintaining weight-loss efficacy, but the panel was careful about how much is known today. Phase 2 signals are not enough. Larger trials will need to show that preserving muscle changes outcomes patients can actually feel.
